About the Role
Join our dedicated team as a Nurse Practitioner in Primary Health. We are seeking a skilled and compassionate individual who is committed to providing high-quality, patient-focused care in a correctional environment. This role offers an opportunity to apply your advanced clinical expertise while contributing to the health and well-being of the diverse patient population.
* Deliver and support high-quality patient care using contemporary advanced practice and expert clinical skills within your credentialed scope of practice.
* Apply specialized knowledge of advanced nursing theory and practice to participate in delivering clinical care directly and indirectly.
* Build and maintain strong relationships within the multidisciplinary team, fostering open communication and a positive work environment.
* Educate, inform, counsel, and support patients and their families with advanced clinical information relevant to their care needs.
* Integrate quality improvement frameworks and contemporary nursing research into all clinical activities to enhance patient outcomes.
* Maintain professional relationships with all healthcare team members, both internally and externally, to ensure cohesive and effective care delivery.

* Comprehensive clinical knowledge within your scope of practice.

* Ability to work as part of a multidisciplinary team to support the delivery of safe, efficient and high-quality care to patients.

* Actively promote a safety culture and follow all health and safety standards and processes.
What You Will Need
* Experience in clinical assessment, care planning, review and discharge planning of individual patients including ongoing regular reviews
* Minimum 2 years full time experience in an advanced practice nursing role
* Current unrestricted AHPRA registration
* Current CPR certification
* Must have full working rights in Australia
* Vaccine Preventable Diseases checklist
* Willing to undergo a police clearance
