About the Role The Therapeutic Care Services Case Coordinator's works from a trauma informed practice and provides effective case coordination, management and support for children, young people through its Therapeutic Care Services – Youth Konnections Division. This is a Full time Position working 76 hours per fortnight and will be based full time at YCSS' Head Office at 110-112 George Street Beenleigh during the training period and then due to the nature of this role, there is no primary location of work and can either be at Level 1, 110-112 George Street, Beenleigh or any other location depending on operational requirements or as directed by the Employer. Duties include but are not limited to: • Participating in On Call on a rotational basis.• Monitoring, directing and advocating for YPs to not only attend appointments but monitor their health and wellbeing outcomes as per the YP's departmental care plans.• Supporting the intake, care management and transition of YP's in care.• Liaising with Department and other stakeholders regarding day-to-day care management matters.• Implementing therapeutic programs and community engagement.• Providing direction, training, feedback to Support Workers.• Monitoring the running of residential houses including but not limited to financial processes of houses that have been assigned to you.• Engaging in roster matching, practice discussion, file note recording, interviews and inductions. Essential Criteria • Over 2 years proven experience in case management support role under human/community services requirements.• Experience in a professional or therapeutic role working directly with complex families and young people.• Hold a Certificate IV or above in a Human Services, Community Services, Youth Services, or related field (Please refer to DCSSDS Minimum Qualification Standards for all Residential Care staff in Queensland).• Hold a valid Licensed Care Services screening check (LCS) or have the ability to complete and pass this screening check – LCS2/LCS7 (Please note that this check can take up to 4 weeks to be processed). • Hold a valid First Aid & CPR Certification.• Hold a valid Paid Blue Card.• Hold a valid unrestricted QLD Driver Licence.• Have a reliable registered motor vehicle with comprehensive car insurance and be willing to transport young people (Vehicle will need to be a minimum of a 4-door sedan to be able to safely transport children and young people if required) in certain circumstances.• Hope and Healing Course (this can be provided by YCSS).
