This position will undertake the role of Assessor for the Commonwealth Aged Care Assessment Program, Regional Assessment program and to provide client-end assessment and support for the Geriatric Medicine Programs.The Aged Care Division delivers a range of clinical and non-clinical services across the entire Local Health District to support Older people in their families to enable them remain supported in their home. The aged care teams and programs have a strong commitment to advocacy, restorative care and reablement back to wellness and independence. The position will enjoy cooperation and support within a strong leadership team and can expect significant challenges in a changing aged care environment which needs to respond to a growing patient population and commonwealth and state health reforms impacting on operations.
