Your part in The Wesley Hospital team Cancer Care CoordinatorWesley Hospital, Brisbane About the Role: The Cancer Care Coordinator plays a pivotal role in providing comprehensive and compassionate care to patients undergoing cancer treatment at Wesley Hospital. This position requires a highly organized, empathetic, and clinically skilled individual to coordinate and facilitate seamless patient journeys, ensuring optimal outcomes and a positive experience. Key Responsibilities: Patient Coordination Act as the primary point of contact for patients and their families, addressing inquiries and concerns. Coordinate appointments, consultations, and diagnostic tests to streamline the patient's care pathway. Facilitate timely communication between healthcare providers, ensuring information is shared effectively. Care Planning and Navigation Develop individualized care plans, considering the patient's specific needs and preferences. Navigate complex healthcare systems to identify and access appropriate resources and services. Provide education and support to patients and their families regarding treatment options, side effects, and self-management strategies. Emotional Support: Offer empathy and emotional support to patients and their families throughout their cancer journey. Identify and address psychosocial needs, connecting patients with appropriate counselling or support services. Foster a supportive and compassionate environment that promotes well-being. Clinical Coordination: Collaborate with the multidisciplinary team, including oncologists, surgeons, nurses, and allied health professionals, to ensure optimal care delivery. Monitor patient progress and intervene as needed to address any issues or concerns. Coordinate referrals to specialists and other healthcare providers as required. Quality Improvement: Participate in quality improvement initiatives to enhance the cancer care experience. Collect and analyse data to identify areas for improvement and implement evidence-based practices. Essential Qualifications and Skills: Registered Nurse with relevant experience in oncology Strong clinical knowledge and understanding of cancer care. Excellent communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Strong organizational and time management skills.
