Job Summary
The support coordinator provides case management activities to meet the needs of clients assigned to the consolidated care team.
This role is responsible for developing individualized care management plans, implementing the plan through service brokering and coordination, and monitoring and evaluating clinical outcomes.
Key Responsibilities
1. Conduct assessments to identify client needs and desires for service delivery.
2. Develop and modify person-centered service plans annually or upon significant medical or social changes.
3. Ensure client choice of providers by providing information for informed decision-making.
4. Maintain confidentiality regarding sensitive material and private health information.
5. Attend required trainings and recertification classes.
6. Collaborate with other professionals to ensure seamless care delivery.
7. Research and develop alternative solutions to client needs.
8. 50% travel to clients as needed.
Qualifications
1. Bachelor's degree in related coursework.
2. Minimum 3 years experience providing case management for individuals with disabilities.
3. Proficiency in Microsoft Office and state-maintained databases.
4. Maintain CPR/First Aid certification and updated state clearances.
5. Valid Driver's License with personal vehicle insurance coverage.
