Job Description
Clinical Care Coordinators play a vital role in ensuring the delivery of high-quality care services that meet regulatory standards.
Key Responsibilities:
* Oversee the delivery of care services, ensuring they meet safety, effectiveness, and regulatory standards.
* Lead, mentor, and support the nursing team, including managing schedules, conducting performance reviews, and fostering a positive work environment.
* Develop and review individualised care plans, collaborate with multidisciplinary teams, and address changes in resident needs promptly.
* Participate in quality assurance activities, conduct audits, and implement improvements to maintain high care standards.
* Ensure accurate and timely documentation of care plans, incidents, and resident records.
Requirements:
* Experienced Registered Nurse with current AHPRA registration.
* Previous experience within a Clinical Care Coordinator (or similar) leadership position is preferred but not essential.
* Strong leadership, organisational, and communication skills; ability to manage and motivate a team effectively.
* Knowledge and experience of aged care accreditation, regulations, standards, and best practices.
* Compassionate, proactive, and committed to high-quality resident care.
