We are a fully registered NDIS provider that offers a range of NDIS supports to help participants, loved ones, and other important people within their lives to get the best outcome for their lives. This is a family-run business designed to provide services to participants and families looking for a more family-orientated approach to the services they receive. We have a combined experience of more than 25 years working in the disability and mental health sector. The business foundation is created from a strengths-based and client-centered approach which enables our service delivery to focus on the participants and family support needs.
Qualifications & experience
Applicants should hold a NDIS Screening Card and Positive Notice Blue Card (desirable)
Previous experience in the Disability and Mental Health Sector and or similar in Human Services.
Certificate III in Individual Support (Disability) or equivalent
Current First Aid Certificate & CPR Certificate
A valid Driver's License
Covid-19 Vaccination Certificate
Tasks & responsibilities
Assisting with personal care i.e., showering, toileting, and dressing.
Assisting with preparing meals, sleep over shifts (Sleep Over shifts - maximum of 3 shifts per week).
Assist with participant’s physical and mobility requirements in accordance with training provided and the relevant workforce health and safety standard, including transferring of participants.
Facilitate community inclusion and participation, dependent on individual needs, abilities, and preferences, and support participants to establish and maintain relationships and interests through access to community activities, venues, and services.
Comply with organizational requirements including, but not limited to, liaison with management, reporting of service issues, completion of documentation, attendance at training, and participation in meetings.
Liaise with and provide assistance to other Disability Workers, under the direction of the Team Leader.
Abide by the service communication, reporting, and participant program progress practices and procedures, by maintaining appropriate participant records. And work with participants, family members, and other stakeholders in a collaborative and supportive manner.
Recognize and uphold the right to privacy, dignity, and confidentiality of the person being supported and their family.
Work within the policies and procedures of Creative Living Supports and the provisions of the Workplace Health and Safety and other relevant Legislation, including support for injured workers and full participation in return-to-work plans.
