The Opportunity UMFC currently has an opportunity for a Senior Interchange Practitioner to join our team in a full-time capacity. You will play a key role in delivering high-quality, person-centred services to individuals with a disability. You will lead the development and implementation of programs tailored to meet each participant's unique needs, enhancing their inclusion, safety and personal development. You will also oversee intake, service delivery and case closure processes ensuring all activities align with best practice standards and regulatory requirements.

Your Experience A Diploma or Degree in Disability, Social Work, Human Services or related field along with relevant experience is required. Proven experience in leading the development of tailored programs for individuals with disabilities. Strong interpersonal skills, demonstrating empathy, self-awareness, and effective communication with participants and colleagues. Attention to detail and commitment to maintaining high standards of documentation and compliance. A sound understanding of duty of care, OHS and risk management practices.
