About the role
We are looking for an experienced Social Worker to join our skilled multidisciplinary teams to provide social work services across Palliative care, Medical and surgical ward areas. The role will include psychosocial assessment and provision of social work interventions including counselling, discharge planning, case management, advance care planning, assistance with financial and accommodation issues, referrals to community agencies, carer support, family violence risk assessments and guardianship.
Your main duties will include:
Primary focus on delivery of outpatient day therapy group programs
Opportunity for development of inpatient therapy programs
Therapeutic support for Calvary Clinic staff including provision of education and in-service program
What you bring:
Mental health social worker or equivalent with eligibility for AASW registration
Therapeutic group skills
Conflict resolution skills
Sound understanding of mental illness and recovery framework
Ability to work independently and as part of a team
