We are seeking a dedicated Family Support Practitioner to join our Children, Youth & Families team in Bundamba.
What You'll Do:
* Conduct Thorough Assessments: Identify strengths, needs, and risks of children, young people, and families through comprehensive assessments.
* Develop Individualized Care Plans: Work collaboratively with clients and stakeholders to create tailored care plans that meet their unique needs.
* Provide Crisis Intervention: Offer timely support during crises to ensure the safety and stability of children, young people, and families.
* Advocate for Clients: Secure necessary resources and services for clients by advocating on their behalf.
* Foster Skill Development: Facilitate evidence-based parenting programs and workshops to promote positive outcomes.
* Cultivate Collaboration: Work closely with internal teams and external agencies to coordinate care and achieve desired outcomes.
* Maintain Accurate Records: Ensure accurate and confidential records are maintained in compliance with organisational standards.
* Culturally Sensitive Practice: Deliver services that are culturally safe and inclusive, respecting the diverse backgrounds of clients.
What We're Looking For:
* Relevant Qualifications: A degree in social work, psychology, social sciences, or a related field (or equivalent experience).
* Experience: At least one year of experience working with children, young people, and families in child protection, family support, or community services contexts.
* Knowledge: Understanding of child protection legislation and out-of-home care frameworks.
* Skills: Strong assessment, communication, and interpersonal skills.
* Cultural Competence: Ability to work effectively with diverse communities, including Aboriginal and Torres Strait Islander peoples.
* Driver's Licence: Possess a current driver's licence – P2 minimum.
* Certificates: Hold a current National Police Certificate, Working with Children (Blue Card), Child Safety and Personal History Screening Check (LCS) – or be able to obtain.
