About the Role
An exciting opportunity currently exists for a Grade 2 Physiotherapist to join the Community Rehabilitation team on a part time, fixed term basis. The Physiotherapist will deliver high quality person-centred, goal-orientated home and centre-based care within a supportive multidisciplinary team.
Classifications for this position will range from VB1 – VB4 ($88,467 - $101,691 per annum, pro rata).
Your Contribution
* Bachelor of Physiotherapy or equivalent
* Current registration with the Australian Health Practitioner Regulation Agency (AHPRA)
* Minimum of four (4) years post-graduate experience in a relevant field of rehabilitation
* Demonstrated knowledge and application of physiotherapy practice and principles in the area of rehabilitation
* Demonstrated highly developed clinical skills
* Understand and support the principles of Health Independence Programs continuum of care for clients, particularly in relation to individualized, time-limited, goal-centred episodes of care that aim to improve health outcomes
* Commitment to the values and health care philosophy of St. Vincent’s
* Current national criminal history check, or willing to obtain

About the Department
This position is part of the Health Independence Programs (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
St Vincent’s Hospital Melbourne (SVHM) HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.
HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).
This position is part of the HIP Community Rehabilitation team. Community rehabilitation is currently provided from our centres at Kew and Fairfield.
The key aim of Community Rehabilitation is to help people who are recovering from traumatic injury or illness, are frail or chronically ill to regain optimal function and to allow people to maximise their independence.
The programs offer a supportive and dynamic team environment, which focusses on patient-centred care.
