About the Role
We are seeking an experienced Support Coordinator to join our team in Rockhampton. This full-time role requires office attendance for 3 months of training. Following this period, a hybrid work model will be discussed.
Responsibilities
* Support clients and their families in accessing and coordinating services.
* Build relationships with internal and external stakeholders.
* Facilitate the implementation of all supports outlined in NDIS plans.
* Enhance client skills to coordinate support and engage actively in the community.
* Empower clients with a deeper understanding of their plans and how to implement them.
* Be available and responsive to client needs, especially during crises.
* Conduct regular progress reviews of client plans, ensuring timely and accurate reporting to the NDIS.
Skills, Qualifications and Experience
* Excellent interpersonal and communication skills.
* Strong written communication skills.
* Demonstratable problem-solving skills.
* Able to collaborate effectively.
* Understanding or knowledge of the NDIS.
* Experience in support coordination or similar.
* Familiarity with psychosocial disabilities (advantage).
* Relevant qualifications in disability services, social work, or related fields (desirable).
Requirements
* Driver's Licence.
* Blue card/Working with Children Check.
* NDIS Worker Screening Clearance (yellow card).
* NDIS Worker Orientation Module certificate.
