Job Description The Stepped Care Coordinator is a pivotal role which has never been done before!You will play a key role in the delivery of a new collaborative community mental health and AOD care model within one of four local hubs.The position has been designed specifically for the hubs and will manage the process of clients coming through the service (you will be the first contact with the client after referral from the Head to Health Phone Line or via other pathways and navigation to the appropriate service).Responsibilities include:??
