Eyre and Far North Local Health Network is seeking a dedicated Mental Health Consultation Liaison to join the Eyre Community Mental Health Team at Port Lincoln Hospital.
Key Responsibilities:
* Deliver Expert Care: Provide direct clinical mental health care, implementing therapeutic interventions and managing individual cases.
* Support Integrated Care: Offer expert assessments and advice to local clinical teams, ensuring seamless care within a risk management framework.
* Autonomous Decision-Making: Exercise independent decision-making in your area of expertise, ensuring high-quality outcomes for consumers.
* Collaborate: Work alongside multidisciplinary teams to implement holistic care plans.
* Facilitate Discharge Planning: Play a key role in complex discharge planning and hospital avoidance, ensuring smooth transitions to community-based care.
Requirements:
* Qualifications: Registered Nurse with a Graduate Diploma in Mental Health Nursing (or equivalent).
* Experience: At least 3 years post-registration experience in mental health nursing, with expertise in supervision and leadership.
* Skills: Strong communication, leadership, and problem-solving skills, with the ability to foster staff development and drive change.
* Cultural Awareness: Ability to engage with Aboriginal communities in a culturally appropriate manner.
