What's involved – The Clinical Nurse (CN) First Nations Health is an advanced practitioner with a unique understanding of Aboriginal and Torres Strait Islander culture and history and how this impacts a person's relationship with good health. The position requires expert clinical leadership in the planning, delivery and coordination of contemporary, person-centered care within a dynamic health environment.
The Clinical Nurse (First Nations Health) under the leadership of the Clinical Nurse Consultant First Nations
Health will be responsible for:
a) Delivering a range of clinical programs in chronic disease management, wellbeing, lifestyle and recovery to Aboriginal and Torres Strait Islander Health clients including but not limited to Deadly Choice and 715 Health Assessments both clinics and outreach.
b) Providing advanced clinical leadership and education, supporting new and novice Aboriginal and Torres
Strait Islander Health Workers and Practitioners in the Aboriginal and Torres Strait Islander Health Team.
Context and Delegations
* This role reports directly to the CNC, First Nations Health Central West Hospital and Health Service (CWHHS).
* This role has a professional reporting line to the CNC, CWHHS.
* This role will work directly with staff within First Nations Team, and CWHHS.
* This role has no financial and human resource delegations.
