About HIP Austin's Health Independence Program (HIP)
HIP is funded by the Victorian Department of Health and Human Services and is part of the Continuing Care Division.
HIP provides multi-disciplinary services that support people with health problems and complex conditions to optimize their function and self-management skills, enabling them to live independently and well in their homes and community.
About the Clinical Nurse Consultant A/B
The Clinical Nurse Consultant (CNC) A/B works in an integrated role as a member of the HIP Chronic Wound service.
Responsibilities include providing high-standard clinical nursing care for effective patient management of chronic wound issues.
The CNC A/B collaborates with the multidisciplinary team, including medical staff, nursing staff, dietitian, and podiatrist, to provide phone and centre-based appointments to support patients.
Main Responsibilities:
* Provide expert assessment and treatment services for clients with chronic wound issues.
* Support clients to remain living at home and prevent unnecessary representation to the Emergency Department or admission to Austin Health.
