Overview
A great opportunity is available for an experienced Registered Nurse to join our dedicated Country Health Connect team in providing direct person-centred nursing care and individual case management to patients in the community setting.
Working with clients in the community you will provide health promotion and education to patients and carers to improve the health outcomes of each individual across the Eyre and Far North Local Health Network region depending on service needs/requirements.
About You
4. You will be registered or eligible for registration as a Nurse or Nurse/Midwife by the Nursing and Midwifery Board of Australia, and be eligible to hold a current practicing certificate.
5. Demonstrating sound knowledge and an understanding of the role of the Registered Nurse/Midwife within a health care setting, you will bring the ability to work effectively within a multidisciplinary team.
6. You will be creative, innovative and flexible when approaching issues within a health care setting, displaying a commitment to providing consumer and family-centred care.
7. Strong communication skills including problem solving, conflict resolution and negotiation skills
