About the position
Provision of case management, assessment, and ongoing support to minimise the harms of alcohol and drug use to young people from a CALD background, primarily South Sudanese, African, and Pasifika background residing in the Western Metropolitan Region.
Some of your key responsibilities will be:
* The provision of developmentally appropriate and culturally sensitive AOD support to CALD young people within the Western Metropolitan region.
* Initiate and maintain positive relationships with a broad range of stakeholders and develop options for implementing a partnership approach to the provision of service delivery.
* Plan, develop, and establish care pathways for young people from CALD backgrounds residing in the Western metro region.
* Manage a caseload of up to eight clients.
* Partake in community development initiatives.
* Complete comprehensive assessments and care plans.
