Harbison is a registered charity owned by the community of the Southern Highlands. Volunteers founded our organisation in the late 1950s to provide local, independent, high-quality care and accommodation for the aged and disabled in the Southern Highlands and surrounding areas. Today we provide general and specialised care across various settings and are one of the largest employers in the Southern Highlands. We exist to support and nurture older people to maintain meaning and purpose in their individual lives.
The Role
Are you a Physiotherapist looking to make a tangible impact on the lives of others? Harbison is seeking a passionate and skilled professional to join our team and help our residents optimise their independence, mobility, and overall quality of life. This is your chance to work in a role that combines your expertise in gerontological functional assessment, acute rehabilitation, and fall prevention management with a commitment to person-centered care.
In this role, you will be responsible for the following (but not limited to):
* Complete assessments of all residents on admission and evaluate the need for interventions consistent with a person-centred approach.
* Trial and prescribe equipment relevant to residents' needs and goals. Including but not limited to; mobility aids, seating, pressure area care, assistive devices, splints, braces and supports.
* Work with other multidisciplinary team members to achieve the best resident outcomes.
* Evaluate the effectiveness of interventions using appropriate tools and outcome measures.
* Identify areas for improvement and participate in continuous improvement projects.
* Maintain accurate documentation, including mobility assessments, pain assessments, fall reviews, treatment records and progress notes.
* Maintain continual professional development requirements as per AHPRA (Australian Health Practitioner Regulation Agency) registration.
* Deliver education sessions for all employees as required, in terms of chronic and acute injuries and annual training requirements.
REQUIREMENTS
* Relevant tertiary qualifications and current registration with AHPRA.
* Current knowledge of aged care and community funding model, accreditation and the aged care regulatory environment.
* Knowledge and skills in clinical assessment, geriatric rehabilitation, and equipment prescription.
* Excellent interpersonal skills with the ability to adapt to change.
* Ability to plan and implement innovative physiotherapy programs, including group exercise therapy and individualised treatment programs.
* Strong communication, written, organisational and computer skills.
* Hardworking, reliable and the ability to work in a team environment
* The ability to take leadership, work under pressure, prioritise and multi-task
* Highly developed interpersonal and communication skills.
* Ability to work independently, as well as part of a multidisciplinary team.
* Ideally, you will have experience in the Aged Care sector and a working knowledge of the Aged Care Planning and Assessment process; however, this is not essential. We seek a strong customer service and care focus and a "can-do" attitude.
