About the Organisation
Based in Gippsland region, you will be working on a dynamic multi-disciplinary Single Assessment Team. This team assesses elderly, community based clients in face-to-face home visits. Our aim is to provide clients with services that allow them to remain living independently at home and feeling supported and socially connected.
The Clinical Assessor Role
This exciting role involves undertaking comprehensive clinical assessments of older people in their homes. The Clinical Assessor supports their access to needed services.
Duties and Responsibilities
Assessors conduct comprehensive home-based face-to-face or phone based assessments of clients referred via the My Aged Care Gateway. Working with each client, they use the Integrated Assessment Tool to establish goals specific to the client; develop appropriate strategies and actions, including referral to aged care and non-aged care services; and develop a Support Plan.
Requirements and Qualifications
You will need a sound understanding of MAC portal use, knowledge of the IAT assessment tool, HCP/CHSP funding, and hold a tertiary qualification in Allied Health, Nursing or Social Work, with current AHPRA registration or able to apply for AASW membership. Recent prior ACAS /RAS assessment experience is ideal. You should also have flexibility to travel across the region to complete assessments, with a generous travel allowance on offer.
Vaccination Requirements
To work in the Community Care sector, you must have at least three COVID vaccinations and 2024 flu vaccination completed.
Key Requirements
You will have a solid background in Community Aged Care and a strong understanding of the Clinical Assessor role. It is essential to have at least six months direct experience working as a RAS or ACAS Clinical Assessor, or at least two years of Community Aged Care experience, working as a Registered Nurse, Allied Health Professional or Social Worker. RN's with a strong AN-ACC background are encouraged to apply.
