Aboriginal Mental Health Clinician (Community Transitions) sought to support Aboriginal and Torres Strait Islander people with severe mental illness, providing culturally sensitive care and connecting them with community resources. This role is part of a multidisciplinary team working with custodial services, primary health networks, and other organizations to ensure continuity of care. Requirements include a Bachelor degree or higher, current registration with the Australian Health Practitioner Regulation Agency, and the ability to travel to various locations across NSW. The position is funded as a Clinical Nurse Specialist Level 2, Health Professional Level 3, and Psychologist Level. The successful candidate will work in a supportive environment, broadening their treatment capabilities and maximising patient outcomes. They will utilise general case management and discipline-specific skills to ensure appropriate access to mental health care and cultural and psychosocial supports. The Community Transitions Team is a recovery-oriented service promoting positive health outcomes and community participation.
