Our Cardiovascular Ward, Ward 41 offers a complete range of cardiac care including interventional cardiology, cardiothoracic surgery, coronary care and cardiac rehabilitation. We also use the latest techniques and equipment, including minimally invasive surgery. Our cardiologists work in partnership with pathologists, radiologists, physiotherapists and nurses to provide excellent care and treatment to our patients.Your role at St John of God Subiaco HospitalYou willl practice as a Clinical Nurse using the Scope of Nursing Practice Decision making Framework. Supervise and direct nursing care of registered and enrolled nurses and staff under delegation.Maintain and improve the standard of nursing/midwifery care, providing clinical expertise and local leadership to ensure that quality patient care is delivered, and the Core Mission and Values of SJGHS are achieved and upheld.The PositionProvides supervision and acts as a resource to other members of the care team including RNs/ENs/Students and PCAs/PSAs.Plans, performs, facilitates and evaluates delivery of patient care that is in accordance with hospital policies and procedures.Develops and implements quality improvement and risk management programs. Evaluates outcomes of recommended actions and provides feedback.Encourages a multidisciplinary approach to the delivery of patient care.Recognises and responds appropriately to unexpected events of rapidly changing situations.Where appropriate, reports changes in patient’s condition to Duty Nurse Manager and or CN After-Hours, and documents identified variations.Provides appropriate health education to patients and significant others and arranges specialised education as required.Assess social needs of the patient and assists in ensuring these needs are addressed through referral to relevant support services.Ensures patient care reflects best practice and is evidence based.Coordinates shifts as required; ensuring allocation of caregivers to patients is based on caregiver skill mix and patient acuity.Accepts responsibility for the effective management of human and material resources within the unit on a shift by shift basis, working with the Nurse Manager/Midwifery Manager to manage on an ongoing basis.Assists in the performance management of other staff.In conjunction with the NM/MM, develops rosters for the unit that consider unit requirements / patient care needs and caregiver requests.Participates and as directed, leads identified projects within the clinical area of the hospital.Assists the Clinical Nurse Manager in the preparation of operational planning and development of unit budgets.Participates in and provides unit-based staff development, in-service and education sessions including the orientation of new caregivers.Actively participates in own annual performance appraisal setting and achieving objectives in liaison with Nurse/Midwifery Manager and Director. Also assists in the completion of team members’ plans as required.Develops and maintains expert knowledge through learning and participation in research.You will hold current registration with the Nurses and Midwives Board of Australia (AHPRA) and have significant recent post-registration experience and the ability to practice at an advanced level.To succeed you will have well developed leadership and communication skills and the ability to plan, assess and develop team and individual performance.
