Community Support Worker/Disability/Aged Care
Aged & Disability Support (Community Services & Development)
Provide direct care in community, disability/hospitals, aged care, and mental health facilities, including but not limited to personal care, domestic assistance, social support, respite care, shopping, and transport.
Develop and implement care plans based on patient assessments and needs.
Collaborate with healthcare team members, including physicians, social workers, and therapists, to ensure comprehensive patient care.
Communicate effectively with patients and their families to provide emotional support and education on treatment plans and options.
Maintain accurate and timely documentation of patient care activities and outcomes.
Requirements:
Certificate III minimum in Aged Care/Disability Support.
Minimum of 2 years of experience in community/aged care/hospitals or mental health facilities.
Excellent clinical skills, including proficiency in administering medications, wound care, and monitoring vital signs.
Ability to work independently and in a team environment.
Willingness to travel to various locations and work flexible schedules, including weekends and holidays.
