Hours: This position is part time (0.5 FTE) Location: Sonder, Port Adelaide More Information & Position Description via this link: https://bit.ly/4fBR0v1 About the Role: The Closing the Gap Integrated Team Care Program Care Coordinator will be responsible for working collaboratively with patients, general practitioners, practice staff and Aboriginal health services to provide appropriate multidisciplinary care and services for Aboriginal and Torres Strait islander people with chronic conditions. All care coordination functions will be in accordance with the care plan developed by a referring GP, with the Care Coordinator driving the implementation of the care coordination model at the local level, utilising existing infrastructure and partnerships to link patients with services needed. Experience Requirements: Demonstrated experience working within a fast-paced environment. Experience in working with Aboriginal communities and the ability to provide culturally appropriate support. Demonstrated experience in working effectively with people with a chronic disease Ability to capture and share clinical information with relevant health care providers, including in electronic format Have a good understanding of the local health system including referral pathways Qualification and Other Requirements: Current or willing to obtain a Working with Children's Check (WWCC) Knowledge and experience in using the Microsoft suite of software and databases. The Care Coordinator will be a health professional with a clinical understanding of chronic disease.
