About us: The Aged Care Assessment Team (ACAT) is a multi-disciplinary team of Nursing and Allied Health Clinicians whose main function is to assess frail clients 65 years and over and Indigenous clients 55 years and over for permanent residential aged care, respite residential aged care, Home Care Services in the community, Transition Care Programme, Short Term Restorative Care. About the role: The Clinical Nurse works across the Aged Care Assessment Team completing ACAT assessments in hospital or community settings according to Commonwealth guidelines and the Aged Care Act 1997, bringing a Clinical Nurse focus to the multi-disciplinary team and to client care, and referring frail aged clients to appropriate service provision. Undertake ACAT assessments (both community and hospital) following ACAT's very specific policies, guidelines, and legislation. Advocate for clients in regard to the provision of community services. Provide specific nursing knowledge to the wider Multidisciplinary Team Clinicians during case conferencing. About you: Mandatory qualification/s: Appointment to this position requires proof of qualification as a Registered Nurse with the Queensland Nursing Council and maintain the registration requirements with the Australian Health Practitioner Regulation Agency, including any necessary endorsements.
