About the Role The Clinical Care and Coordination Clinician (Triple C Clinician) will play a key role in the provision of clinical care and coordination for people with severe mental illness, where care can be managed appropriately within a primary care setting.The Clinical Care and Coordination program (Triple C program) aims to deliver therapeutic mental health services and coordinate and deliver better integrated care for individuals experiencing severe and persistent mental illness, where multiple health care providers are involved in improving clinical outcomes. Primarily, consultation and liaison will occur with primary healthcare providers, acute health, emergency services, rehabilitation and support services, family, friends, other support people and carers and/or other agencies that have some level of responsibility for the client's treatment and/or wellbeing. Candidates with relevant qualifications in Social Work, Occupational Therapy, Mental Health Nursing, Registered Nursing and Registered Psychologists are encouraged to apply.
