About the Role As a Care Coordinator you will oversee and enhance the delivery of our Aged Care services in Rockhampton. Some of your duties will involve processing referrals, aiding clients in commencing with our services, and continuously adjusting care plans to suit their evolving needs. You will coordinate home visits to ensure the needs of the people we support are met and that we are working in partnership with them and their families. Additionally, you'll contribute to program coordination by assessing, improving, and overseeing operational and administrative systems. You will provide a high level of service to ensure our clients and their families feel cared for and supported. This is a 12 month full time, maximum term contract, based at our Rockhampton office. Your week will involve working from the office and traveling to clients' homes as needed. Key Responsibilities Undertake comprehensive client assessments while developing client profiles and care plans in collaboration with the client/representative and other relevant stakeholders Ensure care plans are monitored and reviewed on a regular basis to align with the changing needs of clients, including regular client home visit Collaborate effectively with internal stakeholders to ensure clients’ needs are met including our Rostering team and Practice Leads Develop strategies and/or broker services which will address the identified needs of Aboriginal and Torres Strait Islander clients in consultation with families and communities Represent Life Without Barriers at external forums and meetings, promoting the organisation within the community Skills & Experience Demonstrated Aged Care clinical knowledge with strong administration skills, and ability to work with individuals, families and/or stakeholders Understanding of Commonwealth Home Support Programme, Home Care Packages and My Aged Care Standards High level competency in utilising a variety of technology including mobile devices and computers, proficiency with Microsoft Office, Outlook and database systems High level communication, influencing, liaison and negotiation skills and established problem solving and conflict management skills.
