About the Role Providing support to people with disability living in Chifley you will have a range of experience across disability support including working with in a Group Home/Support Independent Living setting with behaviour support plans, personal care, manual handling and confidence in driving a wheelchair van. Joining our Supported Independent Living (SIL) team on a permanent part time basis, you will support people with disability in shared housing or in their own home, providing various supports including personal care and daily living. You can expect to work across a 24/7 roster. Successful candidates must be able to commit to one of the set fortnightly rosters below: 39.5 hours per fortnight: Week 1: Wed 2:30-8pm, Thu 3:30-8pm, Fri 9am - 2pm and Sun 6am - 2pm Week 2: Mon 3:30-8pm, Tue 6am - 2:30pm and Wed 6-9:30am 40.5 hours per fortnight: Week 1: Mon 3-8pm, Tue 7am - 3pm and Fri 3:30-8pm Week 2: Tue 3:30 - 8pm, Wed 6-9:30am, Thu 3:30-8pm, Fri 9am - 2pm and Sun 2:30 - 8pm 28.5 hours per fortnight: Week 1: Tue 3:30-8pm, Wed 2:30-8pm, Thu 6-9:30am and Fri 2:30 -8pm Week 2: Thu 6-9:30am and Fri 9am - 3pm 35.5 hours per fortnight: Week 1: Tue and Wed 2:30-8pm Week 2: Mon 2:30 - 8pm, Tue and Sat 2-8pm and Sun 9am - 4pm Key Responsibilities Follow person-centred support and behaviour plans Facilitate client goal achievement through meaningful activities Maintain thorough record keeping Build positive relationships with clients, families and colleagues Advocate for client choice and control Skills & Experience Experience working with people with disability in a SIL Setting Previous experience working with behaviour support plans Confidence in driving a wheelchair van Manual Handling and Personal care experience An unrestricted Australian driver’s licence A current First Aid and CPR Certificate Successful candidates will be required to clear probity checks including a National Criminal History Record Check and obtain an NDIS Clearance.
