Clinical Nurse Specialist - Community Rapid Response ServiceDepartment of Health Tasmania – Ulverstone TASThe North West Community Rapid Response Service is seeking dedicated nurses to join our team as a Clinical Nurse Specialist.The Clinical Nurse Specialist - Community Rapid Response Service (CNS):Is responsible for clinical leadership in the development, provision and evaluation of the Community Rapid Response Service (ComRRS) in the North West (NW) Community Health Nursing Service that ensures optimal patient/client/family and community outcomes, under the supervision of the Nurse Unit Manager, Nurse Practitioner and CNC.Provides nursing expertise, case coordination, nursing management and education in the field of chronic/complex care to the ComRRS team, community nurses and to people in their homes who have been referred to ComRRS.Provides expertise and support in the management and care of people with illness, injury or exacerbation of a chronic health condition, within the community.In the absence of the Nurse Practitioner and CNC, is responsible for clinical leadership, decision making and care planning, within their scope of practice and experience, and in partnership with the person's General Practitioner (GP).The Team: ComRRS NW is a community-based service designed to help clients avoid hospitals by collaborating closely with their GPs to provide essential care in the home. Led by Nurse Practitioners (NP), the service operates seven days a week for 0730-2130, delivering timely, complex, and client-centered care throughout the North West Coast of Tasmania.ComRRS NW provide a rapid acute response to those in the community with an acute illness, injury or exacerbation of a pre-existing complex/chronic condition.Work with a dynamic team consisting of NUM, CNC, NP's, CNS's & RN's in close collaboration with local GP's in a hospital avoidance model providing care to clients in their homes (including RACF's) and community clinics.No night-shift, the service operates seven days/week 0730-2130.Details of Appointment: Permanent, full time and part time, shift-worker position working up to 76 hours per fortnight, commencing from December 2024. *notwithstanding hours may be negotiated with the successful applicant.Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or fixed term full time, part time and casual vacancies.Salary: $103,466 - $109,165 pro rata, per annum. Our Employer 11.5% superannuation contribution is on top of this amount.Salary range is in accordance with Nurse and Midwives (Tasmanian State Service) Agreement 2023.You'll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.Post Graduate Allowance of up to 10% based on qualification type.Successful applicants will be required to meet the essential criteria:Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse.Current Driver's Licence.*Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment.
