Key Responsibilities: Assessing the individual needs of clients and developing person-centered care plans.
Coordinating and managing the delivery of in-home care services.
Conducting regular reviews and ensuring care plans are updated as needed.
Acting as the primary point of contact for clients and their families Collaborating with other healthcare professionals and key stakeholders to ensure comprehensive care.
Managing a caseload of clients, ensuring their needs are met in a timely and effective manner.
Monitoring the quality and safety of care services provided.
Advocating for clients to ensure they receive the appropriate services and support.
