We are seeking an experienced Palliative Care Nurse to join our Palliative Care team as a Case Coordinator on a fulltime, fixed term 12 month contract in Morphett Vale. Use your passion for palliative care and hands-on leadership to ensure our dedicated team provide a quality service and make a difference to the lives of our clients and their families.
Key Responsibilities
Provide clinical leadership and support a team of Registered and Enrolled Nurses in a community setting
Coordinate resources to ensure quality person-centred care
Oversee clinical and other care needs for community Palliative Care clients, including complex clinical issues
Work closely with other teams to ensure effective delivery of palliative care services
Identify areas of development within your team, and provide mentoring and education
Use excellent communication skills to build relationships with clients, team members, and key stakeholders, including external care providers.

Current AHPRA registration, with experience in & a passion for Palliative Care
Demonstrated experience in leading a team and coordinating resources
The ability to make informed decisions and problem solve
A strong team focus with the ability to also work autonomously
An understanding of care delivery requirements within a community setting
A current Australian drivers' licence
