Our Chemist Warehouse Chadstone Distribution Centre (DC) has a dedicated department to provide full scope pharmacy and medicine management services to aged care facilities and their residents across greater Melbourne.
We are currently recruiting for an Aged Care Pharmacist to join the team and support this important work. The position is available on a permanent, full-time basis.
About the Role As an Aged Care Pharmacist, your primary accountability will be to provide daily medicine supply services to residents in aged care facilities we support. You may also be required to undertake other clinical pharmacy services and activities that help to ensure the safe and quality use of medicines within the facilities more broadly.
In this varied and fulfilling role, you will be working in the Chemist Warehouse Chadstone DC packing room with direct lines of communication to our aged care partners. As such, you will have minimal community-facing duties.
As an Aged Care Pharmacist, you will be required to:
Provide exceptional customer service to care facilities, care staff, residents and their families. Oversee the accurate and efficient production of weekly Dose Administration Aids, both sachet and blister pack, for residents of aged care homes. Facilitate the daily supply of unpacked medicines and other pharmacy products for residents of aged care homes. Manage and dispense prescriptions for residents of aged care homes, liaising directly with their preferred medical practitioners if and when new prescriptions are required. Identify, resolve, prevent and monitor medicine use and safety for residents of aged care homes. Provide collaborative and patient-centred care through engaging and working with other healthcare providers and the care facility to achieve the best health outcomes for residents of aged care homes. Provide training, advice, support and supervision to intern pharmacists, pharmacy students and dispensary technicians. Provide medicine information and education to nurses, care staff, residents and their families. Participate in the on-call roster for aged-care facilities (where applicable). Perform administrative tasks necessary for the effective provision of aged care pharmacy services and medicine management in the care facilities. To be considered for this role, you will need the following experience and qualifications (at minimum):
Hold current Ahpra general registration as a Pharmacist (with no conditions) Experience working as a general registered Pharmacist in Australia Whilst some experience working with aged-care patients is not a prerequisite, it will be highly regarded.
