The Institute for Urban Indigenous Health Ltd was established by its founding members to provide a coordinated/integrated approach to the planning, development and delivery of comprehensive primary health care services for Aboriginal and Torres Strait Islander populations within the Southeast Qld Region.
The Goodna Clinic was established in 2014 in response to the large and unmet health and related needs of a large and growing population of Aboriginal and Torres Strait Islander people in the Goodna region.
About the Role
The temporary, 6-month role will provide coordination of supports to Aboriginal and Torres Strait Islander clients who experience complex social and mental health conditions and to work effectively with other social and health practitioners to identify and access appropriate services and supports to meet their needs. This position is embedded in the IUIH Goodna clinic and works collegially with Counsellors, Psychologists and other social health team members along with the Family Wellbeing Service ensuring optimal client outcomes.
Your Day-to-Day Duties May Include:
* Maintain SHP Intake Register and manage Wait Lists for Social Health Practitioners
* Support the intake and assessment process and being a central point of contact and access for clients, particularly those with multiple, complex presenting issues and the provision of support to acute/crisis presentations (either in a clinic or outreach capacity)
* Undertake comprehensive assessment of clients’ psychosocial needs in consultation with the client and work with the client to develop a strengths focused plan that sets out priorities and actions, including defining support/interventions from other SHP or external providers
* Manage a caseload of complex clients ensuring systematic contact and follow up with clients to ensure priorities and actions are met, in conjunction with other team members (e.g. psychologists, counsellors, team leaders) to enhance levels of support and case support planning
* Assist in the coordination and delivery of services for clients identified with drug and alcohol and/or mental health problems, in accordance with a case management plan and/or team care arrangement
About You
* Current Blue Card
* Current C Class Drivers Licence (Qld)
* Current Criminal History Check or willing to obtain
* Current CPR Certificate
* Minimum Diploma of Health Services / Social Service / Social Sciences or relevant qualification
Desirable, but not mandatory:
* Demonstrated experience working with Aboriginal and Torres Strait Islander people and/or communities
