Tree of Care is a disability services organisation servicing primarily the Northern suburbs of Melbourne. We are a registered NDIS provider with the following services: Community participation, Support coordination, High intensity support, Community nursing, SIL, Respite Care & Centre based activities.
About the Role
We are currently seeking a highly motivated and compassionate Support Officer to join our team. As a Support Officer, you will play a crucial role in enhancing the lives of individuals with disabilities by providing personalised care and support tailored to their unique needs and goals.
Roles & Responsibilities
* Assisting clients and their family/caregivers with activities, such as self-care, skill development and community participation to achieve goals that are outlined in their plans.
* Assessing clients' needs and planning, developing and implementing educational, training and support programs.
* Supporting families of the client through education or liaising with other services.
* Assessing community need and resources for health, welfare, housing, employment, training and other facilities and services.
* Participating in individual care planning.
* Monitoring and reporting on the progress of clients.
* Ensuring all compliance paperwork for participants is completed in all shifts, taking necessary actions to maintain compliance standards.
* Providing companionship, support and building mutually respectful relationships with clients.
Requirements
* Qualification: At least a Diploma or Higher qualification in Community Services or a related field is a must.
* Experience: At least one year of previous experience as an Aged or Disability Support Officer.
* Police Check: A valid police clearance or criminal record check is usually required to ensure suitability for working with vulnerable populations.
* Key Skills and Attributes: Ability to communicate sensitively and appropriately with and understand the needs of all clients and their caregivers. Other important attributes include flexibility, reliability, problem-solving skills, and a commitment to promoting independence and inclusion for people with disabilities.
