About the role:
We are seeking a dedicated and experienced Support Coordinator to join our team in the Sunshine Coast region, commencing immediately. This position will commence on a part time basis of 15 hours per week and grow as the caseload increases. As a Support Coordinator, you will collaborate closely with clients to optimise their NDIS-funded supports, connect them with appropriate service providers and Allied Health professionals, and monitor their progress toward achieving their plan goals.
Responsibilities include:
* Assisting clients and their families in accessing and coordinating services.
* Developing collaborative relationships with internal and external stakeholders.
* Facilitating the implementation of all supports outlined in NDIS plans.
* Enhancing clients' skills to coordinate support and actively engage in their communities.
* Empowering clients with a comprehensive understanding of their plans & how to implement them.
* Conducting regular progress reviews of client plans, ensuring timely and accurate reporting to the NDIS.
About you:
* Excellent interpersonal and communication skills.
* Strong written communication abilities.
* Demonstrable problem-solving skills.
* The capacity to collaborate effectively.
* An understanding or knowledge of the NDIS.
* Minimum 2 years experience in support coordination.
* Familiarity with psychosocial disabilities.
* Current NDIS Worker Screening Check & Working with Children Check (or willingness to obtain).
* Current First Aid and CPR Certification (or willingness to obtain).
* Completion of NDIS Mandatory Training (or willingness to complete).
* A valid Driver’s License and reliable, comprehensive insured vehicle.
* Relevant qualifications in Disability Services, Cert IV in Mental Health/Mental Health Peer Work.
