Social Worker Casey Hospital – Grade 2 fixed term (parental leave position), 9 day fortnight!
We are now looking to appoint a fabulous Grade 2 Social Worker at Casey Hospital in the adult acute space. You will have opportunities for amazing professional development, career growth and be a part of an awesome and supportive social work team at Monash Health.
A Day in the Life:
As a Monash Health employee, you will maintain a high level of communication with patients, visitors and colleagues enabling you to deliver exceptional care. As a member of the team, you are integral to the Monash Health Social Work service delivery and consistently demonstrate our core values. You will be part of a flexible workforce with personalized support.
You will assist adult patient cohorts from general medicine, general surgery, emergency department, subacute, women’s and children’s and other specialty areas as required. This role will focus on psychosocial assessments, risk assessments, complex discharge planning and family support within the adult acute wards.

* You are a qualified and experienced Social Worker providing a strong focus on patient centred care.

* Experience in a hospital or other health setting is essential.
What you Need:
* An AASW accredited Bachelor degree or entry level Master’s Degree of Social Work that provides eligibility for membership of the Australian Association of Social Workers.
* Experience in a healthcare setting.
* Good organisational skills and a positive, enthusiastic attitude towards work.
* Experience in working with culturally and linguistically diverse communities.
* Able to work autonomously.
* A friendly, outgoing and reliable demeanour.
* Well-developed clinical assessment and complex discharge planning skills.
