NHW is excited to offer this new senior clinical role for a highly experienced Registered Nurse and/or Allied Health Professional who will be responsible for providing clinical leadership to SACS and TCP programs, along with an agreed clinical caseload.
The successful applicant will work closely with the Manager of Health Independence Programs and Transitional Care Program and will require personal attributes that foster collaboration between staff, departments at NHW and relevant external services.
The coordinator will need to be highly motivated and demonstrate knowledge and experience of working within community programs.
