About you:
In your new role you will:
Act as a cultural liaison to advocate, liaise, network and refer to appropriate Non-Government Organisations, Aboriginal and Torres Strait Islander Liaison Officers and the Multidisciplinary Team in collaboration with and under direction of Registered Nurse.
Provide effective support and advocacy for identified Aboriginal and Torres Strait Islander High Risk Foot and or Diabetes clients by conducting timely home visits, case management/conferencing, follow-up and referrals to relevant health services/programs.
Deliver evidence-based care by working collaboratively and in partnership with consumers, carers, external agencies and other health professionals.
Maintain accurate and timely documentation through the provision of monthly activity reports, client file documentation and monitoring Aboriginal and Torres Strait Islander identification records.
