Are you an experienced Case Manager, committed to delivering early intervention strategies that support young people and families?This is a part-time role, 8 days/fortnight (60.8 hours), contract to 30/9/2025.About The Opportunity:Part of Lutheran Services Intercept at Caboolture is a not-for-profit community organisation that provides early intervention and prevention services for young people and their families in the Moreton Bay region.Intercept delivers targeted support programs, unique to young people and their families’ individual circumstances, that support them to achieve their goals/improve quality of life.Underpinning our evidence-based service design are our Lutheran Services values and Model of Care and Service. Our team is warm and friendly and we live our values at work every day.This opportunity will work with two programs:Empowering Family Connections (EFC) - a counselling and intervention support program that offers support to families experiencing vulnerabilities with children and young people aged 10 to 18 years.Transitions - a counselling and intervention support program that works with young people aged 10 to 18 years who are subject to statutory intervention through the Caboolture or Morayfield Child Safety Service Centre.About The Role:This position is responsible for providing comprehensive case management and evidence-based interventions to young people and their families/carers within a therapeutic framework. This role involves assessing needs, developing individual support plans, coordinating services, and advocating for clients to promote their well-being and successful outcomes. The Senior Case Manager works collaboratively with multidisciplinary teams and external stakeholders to ensure holistic support for clients in a dynamic and challenging environment and provides specialist technical advice.Responsibilities will include:Assess and review client needs, including the establishment of goals for the intervention period;Use your skills in counselling support, advocacy, complex case management, and mediation for young people and their families;Provide sustainable interventions for clients and their carers ensuring that all casework is underpinned by evidence-based practice;Assist in the development and facilitation of targeted individual and group programs for young people and their carers;Contribute to service planning including the development of work plans, evaluation of services provided and subsequent reporting;Participate in networks and forums to promote, enhance and develop links within the community;Work closely with key agents such as child safety, police, health and educational links.About You:Use your knowledge and demonstrated expertise in this field to ensure holistic support for clients in a challenging environment.Bachelor of Human Services or related degree level qualification in human services delivery and data management systems;Ability to provide multidisciplinary advice and expertise;Knowledge of statutory requirements, program and organisational policies and procedures.Apply now!To view the position description, please click here.About UsWe draw on our rich Lutheran tradition of caring for everyone – the individual, family and the community.
