The Area Coordinator will provide support and supervision for a team of case managers in our Hume community, ensuring clients receive the necessary support and care based on their funded level of package.
Duties and Responsibilities
* Promote a non-institutional philosophy by recognising and promoting the rights of clients
* Promote consistent growth and viability of the Home Support Program (HCP)
* Develop strong relationships with local services and networks
* Ensure duty of care to both clients and staff is adhered to at all times
* Provide high-quality comprehensive case management services
* Support and supervise a team of case managers
* Maintain a small caseload of Home Care Package clients
We are Looking for Candidates with:
* An approved tertiary qualification in Social Work, Nursing, Welfare, Community Services, or Allied Health
* Demonstrated well-developed networking skills
* An understanding of the aged care sector and community services
* A current Driver's License
* Experience in case management supervision and staff development
* Experience in service delivery in a community setting
* Sound knowledge of the aged and community services legislative frameworks
* Experience in the Health or Aged Care sector and Homelessness
* A commitment to the organisation and its philosophy of social justice
