We have over 1,500 employees and partner with government, businesses, and not-for-profits to provide people in Australia access to a range of health care services and have launched over 100 health programs with partners in the last decade.Your OpportunityAs a Clinical Care Coordinator (CC), you will be responsible for the coordination and provision of consumers' goals and services under a consumer-directed care model to Home Care Package consumers to promote the individual's health and wellbeing and enable them to remain living independently in their own homes.This position will see you visit consumers' homes in the community to undertake comprehensive assessments and regular reviews of your consumers, as well as provide virtual, technologically-based coordination to consumers.You will also have the opportunity to showcase your outstanding administrative and case coordination skills and knowledge within the office, supporting with clinical review of documentation and road-staff handovers, clinical escalations.You are at the heart of the consumers' experience with Home Health, and central to this is your strong ability to develop and maintain effective relationships with all stakeholders. In addition to being organised, you possess strong judgment and communication skills which are central to your ability to work autonomously.What will set you apart from the rest?Working in this autonomous position, you will have a passion for working within the community aged care setting, attending to individualised assessments and creating personalised goal-oriented supports and services. You will have an organised and flexible mindset that allows you to meet the unique needs of your consumers. You will be consumer and goal-focused, who is driven to achieve positive customer outcomes together with:Recent experience in Aged Care CoordinationExperience within community nursing/home care packages sector is highly desirableThe ability to multi-task and prioritiseExceptional communication skills and the ability to build a rapport with consumersStrong administration skills and the ability to ‘think outside the square’Excellent computer skills, particularly Excel, and the ability to navigate systemsAbility to work autonomously but contribute to a supportive and engaging teamWillingness to use your own car to travel throughout metro Adelaide is required with kilometer reimbursement fortnightly.This role is Monday to Friday. The successful applicant(s) will need to be flexible with hours; shifts required to work are 7am to 3pm, 10am to 6pm or 12pm to 8pm.
