Join us in shaping healthier futures. Together we can do more. We are seeking an experienced Clinical Nurse Specialist to join our Aged Care Outreach Service team. As the successful candidate, you will play a key role in providing timely, high-quality care to residents in Residential Aged Care Facilities (RACFs). The role will focus on offering acute care treatments at the residents' homes, enabling them to remain in their familiar surroundings while receiving the best possible care.
Your responsibilities will include:
Assessing, diagnosing, and managing acute deteriorations in RACF residents.Reducing emergency department presentations.Promoting hospital avoidance strategies to minimise complications.Upskilling RACF nursing staff through education on common aged care conditions.Developing care pathways for recognising and responding to acute deteriorations.Collaborating closely with GPs and other healthcare professionals to ensure continuity of care.Facilitating direct hospital admissions when required, bypassing the emergency department.Join us in making a significant impact on the quality of life for older adults and improving their access to specialised care where they live.
