Aged & Disability Support (Community Services & Development)
An exciting opportunity has become available in our Home Care (Aged Care Services) team, managing staff and clients across Home Care Packages. We are looking for a permanent full-time Relief Home Care Coordinator to report to the Home Care Team at our Currambine hub.
The successful candidate will be responsible for the delivery of services to Home Care clients, including government funded HCP and occasionally CHSP Flexible Respite and private clients. Your role will include the development of care plans in partnership with your clients, coordinating all client care, liaising with Mercy Care’s clinical and allied health teams, and managing your clients’ budgets. You will work closely with clients, their family, and their support networks to achieve positive outcomes.
When other Home Care Coordinators are on leave, you will support a team of Support Workers, Senior Support Worker, and Home Care Assistants. This will include providing guidance, mentoring, and training for staff, providing learning opportunities and development, and supporting staff to deliver quality care that promotes client choice and independence.
About you
In addition to having experience in the similar industry, you will also have:
* A Certificate IV in Community Services / Aged Care / Disability, or a Health / Human Services equivalent tertiary qualification which is preferred.
* Experience guiding staff and managing home care packages or significant frontline experience.
* Demonstrated knowledge of service provision, allocation and compliance with contractual obligations to ensure quality service.
* Knowledge of the Aged Care Quality standards.
* Well-developed communication and interpersonal skills.
* Demonstrated time management and organisational skills to effectively plan and prioritise work tasks.
* Demonstrated empathic and flexible approaches to manage the sensitivities of conflict resolution of consumer / employee issues.
* Proficient in the use of the Microsoft suite of programs including Outlook, Word and Excel with experience using Client Record Management Systems.
* Current Basic First Aid Certificate (or ability to obtain).
* Current C Class Driver’s Licence.
* COVID and Influenza vaccinations are preferred but not required.
* National Police Clearance (obtained within the last six months).
