About the opportunity This position will be responsible for assisting people with disability alongside their family or carers in strengthening their capability to implement supports in their NDIS Plans or other funded life plans to participate fully in the community. This position will be focused on Brisbane North and Northeast Suburbs. As a Senior Support Coordinator, this position incorporates some managerial tasks and activities to support the team. This role provides a mix of supports to increase participant’s capacity to maintain relationships, manage service delivery tasks, live more independently, and be included in the community. This is a Permanent Full-time position. About the successful candidate To be successful in this role, candidates will have substantial experience in providing support coordination for people with disabilities. We are looking for: A self-starter, self-motivated person who can work independently and takes pride in delivering high quality services. Relevant tertiary qualification in Social Work or Human Services or similar and technical knowledge and practical work experience supporting people with a psychosocial disability or mental health conditions. Knowledge of NDIS Legislative Framework and Support Coordination Processes. Maintaining partnerships with key stakeholders and networks relevant to the organisations’ supports and services, that strengthens our community presence. Respond positively and adapt to a rapidly changing environment, identify, and manage potential and real risks which ensure the personalization of supports are risk smart and informed. Proven ability and initiative to manage concerns, complaints, grievances, critical incidents, and other service delivery matters (with delegation) to improve all aspects of operations. Lead a team of Support Coordinators and Service Delivery Strong time-management and administration skills Strong experience using Office 365 products (incl. Outlook, Word and Excel) As part of our recruitment and selection process, all SIRV employees must have or be willing to undertake a range of verifications including a NDIS Worker Screening Clearance, Blue Card and a National police check.
