About the Role The Lockyer Valley Housing Service (LVHS) Program is funded by the Queensland State Department of Housing to provide direct services to households' homelessness or at risk of homelessness through early interventions, coordination of and advocacy to access appropriate services to sustain existing tenancies or transition to a suitable tenancy option.
The aim is to maximise people's capacity to be independent, self-reliant and connected to appropriate social and community supports including assisting them to stabilise housing options.
The Support Worker position provides high quality customer first service experience through initial assessment and triaging of needs, case planning and delivery of direct services, providing warm and successful referrals to other services and working in collaboration with the team to deliver holistic interventions and a coordinated response with stakeholders for ongoing needs.
The components of the program include centre-based access and support, mobile support, immediate and transitional accommodation and will provide direct service provision, coordination and advocacy of services with individuals and family units both remotely via use of technology and in their own homes or through outreach.
Utilising the WMQ Housing & Homelessness Practice Framework, WMQ Model of Care and Wesley Charter, service delivery is person-centred, proactive, reliable, comprehensive, holistic, practical, and delivered in a manner that is respectful of the dignity and privacy of individuals.
The role sits within the WMQ Housing and Homelessness Services department and will have opportunities to work across similar program areas and locations.
Key Role Accountabilities: Determine eligibility through conducting assessments with families and individuals from the stated client group.
This is to identify both needs and risks in the aim of providing a targeted and needs based intervention.Provide holistic brief intervention support and advocacy for the client(s) as assessed to meet their needs.
This includes developing, planning, implementing and reviewing holistic case management plans with client(s) and provide tenancy, information, advocacy and support to clients as required.Maintain and update resources and information required to support service delivery.Ensure that activities meet duty of care obligations under the provisions of health and safety legislation and WMQ policy.Facilitate processes to optimize communication, case coordination, access to services and linkages between other service providers, community partners and relevant stakeholders to ensure client(s) needs are met.Ensure the needs and outcomes for the target group and program objectives are clearly understood by stakeholders and report on barriers being experienced to access service delivery.Drive a customer centric culture that maintains their needs at the heart of all activities.Identify opportunities to provide cross disciplinary services which may be of benefit to the customer.Undertake all administrative duties including but not limited to ensuring high quality, up to date case management notes/case plans/materials/statistics/reports and maintenance of accurate data.Align one's workplace behaviour in accordance with the WMQ Model of Care, the Wesley Charter and Wesley Mission Queensland's Vision, Mission and Values.Work according to team values, and display the attributes of conscientiousness, adaptability, optimism, kindness and reflective practice.Work collaboratively within a team environment, actively participate in regular staff meetings, case reviews, training, and supervision and ensure continuity of service capacity.Supervision and support of volunteers and placement students as required.Operate within the financial delegations and budget and maintain appropriate receipts and accounts.Accept other relevant duties and responsibilities as determined by the organisation in the delivery of activities and services.About You Tertiary qualifications in human services or a related field, with demonstrated experience in the sector.Demonstrated experience in conducting risk and needs assessments in an intake, case planning and coordination, advocacy and referrals or similarly related position.Demonstrated experience working with families and individuals at risk of homelessness or homeless using trauma informed, person centred case management principles in an identified framework or model of practice.Demonstrated high level communication and interpersonal skills including experience in engaging and working with diverse people with varied needs, including but not limited to tenancy issues, mental health, domestic and family violence, financial stress, substance use, home management and barriers to employment/education.Demonstrated ability working within a values-based organisation, supporting positive team functioning, working collaboratively, flexibly and respectfully, fostering inclusion and diversity, maintaining a clear professional practice framework and professional identity.Understanding and commitment to social justice principles, confidentiality and privacy.Demonstrated ability to work under general direction with established procedures and guidelines and as required with minimal supervision to achieve client and program goals.Experience in establishing and maintaining agency network relationships within the human service environment to better meet client needs.Demonstrated high level computer skills with the Microsoft Office suite of applications and customer databases including practice management systems.Current Suitability Card for Child Related Employment ("Blue Card").Current First Aid and CPR certificateCurrent Mental Health First Aid certificateWhat makes a difference for us: Proof of Vaccination Status as required by Legislation, Health Directives or WMQ Policy throughout employment and prior to commencement.Relevant probity checks required by legislation and WMQ policy.Current Driver's Licence and willingness to drive in the course of work.Why Join Us We value diversity, strive for inclusion, and come together to walk alongside those in need to provide hope and compassion.
