About the opportunity
With our organisation's expansion, we are excited to offer an opportunity for an experienced Coordinator – Individualised Services to become part of our current team at the leading disability support organisation in the Goldfields region of Kalgoorlie. This position is offered on a part-time basis.
Within this role, you will be responsible for managing a range of support services to assist people with disabilities to live the “good life”. Key responsibilities include:
* Ensure positive relationships are developed and maintained with the people we support and the relevant stakeholders in their life in order to implement and continuously improve their individualised support services.
* Encourage and promote choice and control for the people we support.
* Facilitate opportunities for people to be valued members of their community whilst ensuring strategies within their Individual Plan are aligned with their goals and aspirations to achieve their desired outcomes.
* Provide leadership and mentoring to support staff who come from varied cultural backgrounds with a range of different life experiences.
Essential skills and experience
* Ability to build and nurture relationships at a personal, organisational, and community level.
* Effectively manage and implement medically developed care plan requirements as directed by qualified professionals and specialists.
* Demonstrated computer skills including working knowledge of databases, Microsoft Excel, and Microsoft Word.
