About the role:
We are seeking an experienced Community Support Worker to support our Community Transition Care Program (TCP) across the Metropolitan area.
The Juniper Transition Care Program is a goal-oriented, strength-based, reablement program. The program goals are created in partnership with the customer and family and outcomes are measured against the progress achieved towards goals. Progress towards goals and evaluation of care and interventions occurs through a collaborative approach by the multidisciplinary team. Care plans are revised frequently to support progress towards independence.
Whilst working in this rewarding role you will provide support and assistance to enable clients to maximise their independence in their own homes within the community, incorporating the reablement approach. Care and services are to be responsive, timely and empowering, enabling the client to lead a continued safe and independent value added life within their own home.
This position requires the delivery of personal care to our TCP clients, and our HCP/CHSP clients as required. To be considered for this role you must have at least 12 months experience as a full Community Support Worker to the aged, and this must include competency in the administration of medications.
On offer for the successful candidate is a permanent full-time position working 75 hours per fortnight. To be considered for this role you must have full availability and flexibility to work shifts that may require coverage Monday to Sunday with a mix of morning and evening shifts. Flexibility is required to meet demand changes at short notice.
Key Responsibilities:
1. Personal care, including showering, dressing, continence care
2. Assist Client with administration of correct medication
3. Assisting high care Clients with activities of daily living
4. Assisting Clients with domestic duties, meal preparation and other household tasks to ensure safe and hygienic living conditions in their own homes
5. Social Support, including transport to shops or appointments
6. Adherence to Client Care Plan
Key Requirements:
7. Current Australian driver’s licence and reliable car - essential
8. Past experience working in Aged care as a Community Support Worker is essential and this must include the delivery of personal care
9. Medication competency
10. Certificate III Aged Care - desirable
11. Dementia Care experience - desirable
12. Current First Aid - desirable
