Working Wednesday, Thursday and either Monday, Tuesday or Friday
- Position located primarily at the Fitzroy site, however clinicians may be required to work across other sites within the Health Independence Program, and may be required to travel for regional clinics within Victoria

About the Role
Your role will be to provide high quality Social Work assessment and management for clients referred to Polio Services Victoria, many of whom come from diverse cultural backgrounds. Additionally, your role will include:
- Collaborative work with the multidisciplinary team members, referring professionals, community professionals and other key stakeholders within a flexible, multidisciplinary structure.
- Provide ongoing education and advice to clients and health professionals regarding psychosocial management of people with a history of contracting polio.
Duties
- Provide clinical leadership in Social Work practice for PSV
- Drive best practice based on evidence and demonstrated outcomes
- Delivery of accountable, client focused, high quality social work services - assessment, care planning, referral management, reviews, counselling and carer support to all clients within your caseload
- Delivery of groups with clients in collaboration with team members.
- Contribute to service and stream development
- Lead at least one quality activity within PSV
Classifications for this position will range from SC31 - SC34 ($102,091 - $116,646 per annum, pro rata for part time)
A Bit About You
- Bachelor of Social Work
- Minimum of seven (7) years' post-graduate experience
- Eligibility for membership of relevant discipline association
- Knowledge and experience regarding the delivery of services within the disability sector
- Highly developed interpersonal skills that incorporate the qualities of excellent communication and leadership
- Current Nationally coordinated criminal history check, or willing to obtain

About Polio Services Victoria
Polio Services Victoria (PSV) is a state-wide specialist allied health and medical service for adults with a history of polio who reside in Victoria. The key aim of PSV is to assist clients with managing the effects of the lifelong effects of polio. PSV is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.
