Our organisation is constantly growing, with many homes and thousands of residents and employees across Victoria, New south Wales and Tasmania.Respect also has home care servicesin multiple states, and more than 700 units in differentretirement villages and independent living communities.About The Role: The Clinical Care Coordinator plays a crucial role in coordinating person-centered clinical care for residents, adhering to established policies, procedures, and professional standards.Working in collaboration with the Care Manager, the primary purpose is to concentrate on individual resident needs.This involves ensuring the existence of a current and well-documented Care Plan that effectively addresses residents' needs.The Care Coordinator also contributes to the review and delivery of the Care Plan with the support of the multi-disciplinary care team.Day-to-day reporting for this position is directed to the Care Manager or General Manager.The Clinical Care Coordinator works in collaboration with the Care Manager to ensure effective coordination of resident care, including:Assists with the admission of new residents to the facility, ensuring that they are assessed.Assess the individual needs of residents and develop person-centered care plans in consultation with other healthcare professionals, ensuring that care is tailored to meet their specific needs.Collaborate with other healthcare professionals to deliver comprehensive care to residents.Communicate effectively with residents' families and health care providers, keeping them informed about their health conditions, care plans, and any significant changes or concerns.Maintain accurate and up-to-date resident documentation, including care plans, behaviour charts, medication charts, observations and/or incidents.Provide and assist with direct nursing care to residents, including and not limited to assisting with activities of daily living (ADLs), administering and managing medication, catheterisation, conducting observations and monitoring, and wound management.Actively participates in case conferences and relevant meetingsComplete clinical assessments and reviews at required intervals and escalate as required, including COD status, daily wound care and fall managementUndertake audits as required, including the Schedule 8 drug countAssist with pharmacy ordering as requiredCheck alerts, overdue tasks and progress notes to ensure they are completed effectively and to standardCoordinate infection control measures are in place and ongoing risk reduction strategies are being implementedAssist with the investigation of clinical incidents and their close out as requiredCollate data and prepare information for reports as requiredEssential: Bachelor of NursingCurrent Australian Health Practitioner Regulation Agency (AHPRA) registrationNursing experience within a residential aged care facility, preferably at least 3 years of aged care experience.Experience of working cooperatively with multi-disciplinary care teamsStrong clinical assessment, documentation and practice skillsEffective communication and interpersonal skillsProficient computer and information management skills, including ability to use Microsoft and clinical software systems.Experience in a similar clinical care roleAn understanding of the aged care accreditation processWorking knowledge of quality improvement programsCurrent Driver's License.To be considered for employment with Respect, each candidate must satisfy the legislative requirements of Aged Care Worker Screening to ensure suitability to work in the aged care industry.
