About the program
Wesley Care finder program is designed to assist older people in accessing and understanding the aged care system. Care finders will provide specialist assistance to help connect participants to relevant programs and supports in the community.
Participants in the program may require intensive support due to:
5. isolation or lack of a support person carer, family or representative) who they are comfortable to act on their behalf and/or who is able to support them in accessing aged care services via My Aged Care.
6. communication barriers, including limited literacy skills.
7. difficulties processing information to make decisions.
8. resistance in engaging with aged care for any reason, where their safety may be at immediate risk, or they may end up in a crisis situation within the next months.
9. past experiences that mean they are hesitant to engage with aged care, institutions or government agencies.
This program is funded under Primary Health Network (PHN) Hunter New England and Central Coast.
About the role
We are seeking an experienced Case worker to join us in establishing and rolling out this program across Upper Hunter.
Our Caseworkers will be expected to provide assertive outreach in order to ensure the delivery of a high-quality person-centred service, that will complement, but not duplicate, the aged care channels that provide access support to people who are able to proactively navigate the system for themselves.
Care finder Case Workers will therefore specifically target senior Australians who need intensive support who could otherwise fall through the cracks. This will include people who are not yet receiving aged care services.

Essential criteria
19. tertiary qualification, in a related field, Aged Care, Community Care, Social Work, Allied Health, Nursing and/or demonstrated professional experience working with clients to deliver a community-based service within the aged care sector
20. demonstrated high-level understanding of My Aged Care systems and processes (including aged care assessments) and a sense of aged care services
21. at least 2 years’ case work experience working with vulnerable clients with complex needs
22. demonstrated experience working across a range of service providers developing and maintaining professional relationships and networking across organisations and service systems
23. demonstrated commitment to supporting the needs and rights of people from diverse backgrounds, Aboriginal and Torres Strait Islander communities, Culturally and Linguistically Diverse communities, people from Lesbian, Gay, Bisexual, Transgender and/or Intersex communities, and those from rural/remote communities
24. understanding of trauma informed care and culturally appropriate practice
25. high-level communication and administrative skills, including collecting, recording, and reporting data aligned with key performance indicators
26. ability to work effectively in a team environment as well as independently.
27. understanding of WH&S issues and risk management for client home-based care and during transport of clients
28. current NSW or National driver’s license
29. fully vaccinated against COVID-
30. First Aid Certificate or willingness to obtain.
Remuneration
Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 0, Level 5, with capacity for salary packaging and the provision of a fully maintained motor vehicle available for private use. Superannuation is paid in addition at %.
